The Hub is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use challenges by taking a person-centred, equity focused approach to ensure that clients have access to the care that they need.

The HART Hub operates under the core principles, Low-barrier access, Trauma-informed and culturally safe care, Integrated, wraparound services, person-centred approach, Recovery and stability focus and Collaboration and community partnership.” 

Services are available Monday to Friday, 9am-4pm

Mobile Clinic coming early 2026

The HART Hub Dufferin will begin a phased rollout of services starting on November 17, 2025, with full implementation expected in early 2026.

  • Onsite and outreach support for mental health & addiction
  • Housing help
  • Service information and navigation
  • Counselling
  • Basic needs support (food, hygiene products)
  • Nursing care

  • Transitional and supportive housing
  • Employment support
  • Peer support
  • Foot and wound care
  • Dietary support
  • Mobile clinic services

Immediate Crisis

If you require immediate care or are in crisis, please call 911, go to the nearest emergency department of a hospital, or call a crisis line. Crisis lines provide 24-hour, 7 days a week support to those experiencing distress or crisis, mental health and addiction concerns, family violence, or are in need of suicide prevention services and more.
