How To Apply - Instructions

All applications for housing and support services/programs through Supportive Housing In Peel (SHIP), Housing and Support Peel (HASP), Supportive Housing in Etobicoke/York (SHEY), and Peace Ranch are processed through Central Intake.

To begin the application process, contact Central Intake at 1-905-795-8742. Request an information and application package. Once received, applicants must:

• Sign the consent form
  
• Complete the application form
  
• Arrange for a psychiatric report (which Central Intake must receive within six months of date of application)

Please note, if the Doctor’s Section of the Central Intake application form is not filled out by a psychiatrist-family doctor, you will need to sign the Consent to Release Health Information form. This must be mailed to Central Intake. The original signature is needed for documentation from hospitals and healthcare professionals. Applicants must also send a copy of their income statement and rent receipt (if they are currently paying rent).
