Central Intake is a single point of access for those seeking supportive housing for persons dealing with a persistent mental illness. Central Intake serves the SHIP, HASP, Group Home and Peace Ranch programs in Peel and the SHEY program in Etobicoke/York.
Where can I get an application?
Call Central Intake for a SHIP Application Package. A package can be mailed, e-mailed or faxed upon request. The application package outlines the requirements and provides the necessary forms to be completed. The more complete the application, the quicker it can be processed. Completed applications are accepted via mail, fax or walk-in, although the Consent Form to Release Personal Health Information must be sent by mail or dropped off to the office, as we require the original, if no written documentation confirming the applicant’s mental illness is included with the application.
Who completes the application?
Referrals can come from the individual, families, community mental health clinics, general hospitals, shelters, psychiatrists or other community agencies. However, the completed application and the consent form must be signed by the applicant.
What are the admission criteria?
Anyone can apply as long as they meet the following basic eligibility requirements:
Supportive Housing In Peel (SHIP) serves individuals over the age of 16 who suffer from identified and persistent mental illness, and who require supportive housing but who are not at risk of homelessness. This includes both the independent living program (ILP) and two group homes.
The Homelessness Initiative (HASP/SHEY) serves people over the age of 16 with an identified and persistent mental illness, who are also homeless or at imminent risk of homelessness. An eviction notice or a letter regarding imminent risk of homelessness and income statement/rent receipt are required to be submitted with the application in order to provide confirmation applicant is at risk.
Peace Ranch serves people over the age of 16 who are diagnosed with schizophrenia and require 24 hour support in order to live in the community. Peace Ranch also provides Day Program services for those qualified.
After submitting the application, what is the next step?
The application process has two stages. First, the completed application will be reviewed. The applicant will be notified by mail if they are eligible for the program. The second stage involves a series of face-to-face interviews between the applicant and a support worker. This happens when the applicant has come to the top of the wait list and is intended to further assess their eligibility. At each stage, if an applicant is deemed not eligible, they will receive notification clearly outlining the reasons.
How soon will I receive housing and supports?
Supportive Housing services in Peel and Etobicoke/York are highly in demand. As such, there is a period of time that eligible applicants must wait before being offered housing. This wait period ranges from approximately 1 ½ to 6 years from the date of application. Each wait list for Supportive Housing In Peel and Etobicoke/York is maintained in a chronological order by the applicant’s date of application and will be offered housing based on this date.
Central Intake cannot tell an applicant what number they are on the wait list. Only approximate time estimates can be given. Once the applicant’s name comes to the top of the wait list, they will be notified of the opportunity for face-to-face interviews.
Please note: Applicants will only receive three offers of housing. If these offers are rejected, the applicant will be removed from the waiting list and will need to reapply.
If I am deemed not eligible for the program, can I appeal?
Yes. Supportive Housing in Peel has a comprehensive Appeal Process. This is a system of appeal for applicants wishing to challenge and possibly reverse decisions that refuse service resulting from the documentation review by Central Intake or the face-to-face assessment by SHIP or partners’ support workers.
If my situation changes should I notify SHIP?
Absolutely! It is essential to have current information on file. This helps to ensure success during the application process.
Specifically, applicants must notify Central Intake if:
Is Supportive Housing In Peel the same as Peel Living?
