SHIP recently entered into a partnership agreement with Peel Senior Link and Punjabi Community Health Services to provide supportive and health services to seniors who want to continue living independently in their homes.  


The Integrated Seniors Team Program has created integrated service teams that make in-home visits within North West Brampton. Comprised of healthcare service providers from each of the three partnership agencies, these teams provide risk management assessment and education, personal support services and access to supportive housing options.
 

With a focus on promoting seniors’ health and wellness, the Integrated Seniors Team Program assists:
 

  • Seniors aged 55 and over, with complex physical,  emotional and medical concerns, who want to continue to reside in their home environment
  • Seniors who would benefit from health education, crisis prevention, chronic disease self management and intervention for social isolation


The Integrated Seniors Team Program includes a 24-hour on call system for emerging crisis situations. Individuals are linked to appropriate support services. 


It utilizes the *Principles of Healthy Housing™ to improve wellness and optimize the well-being of seniors. Self-management is supported and encouraged by home visits focussing on the individual’s social, educational, physical fitness and mind stimulation needs.


Chronic disease prevention and management is offered through linkages with existing health programs in the community. Additional benefits include fall prevention training, assessment and management of environmental and home risk factors.
