Case Management

In response to the current supportive housing waiting list in the Region of Peel (Mississauga, Brampton, and Caledon), SHIP provides Case Management to individuals who do not have a formal support system.

The Case Management initiative offers supports, services, and referrals to individuals who are on the waiting list for supportive housing. This initiative is designed to give clients the opportunity to live interdependently.

Case Management support services at SHIP are specific to individuals on the waiting list. Support services may include:

• Individualized assessment and service co-ordination
• Linkages to other services
  
• Assistance in finding suitable housing on a permanent basis
  
• Support during the transition to suitable housing
  

In providing referral to other services, the SHIP Central Intake Department is a resource link to community services, medical services, and open-ended case management providers.
