Assertive Community Treatment

Assertive Community Treatment (ACT) is a multidisciplinary team consisting of professionals in the area of psychiatry, nursing,  social work,  occupational therapy,  substance abuse,  a vocational and peer assistance.

Developed and implemented by SHIP, the ACT team provides client-centered recovery-oriented treatment for individuals who suffer from mental illness.

This multidisciplinary assertive outreach team specifically addresses the needs of individuals with a concurrent disorder or dual diagnosis, transitional youth and seniors, and those who have come in conflict with the law or have the potential to do so.

Linkages to other resources are provided through established connections with appropriate services in the community. Augmented by this collaborative approach with community agencies in Brampton and Caledon, the ACT Team provides:
  1. Assertive outreach
  2. Individualized treatment
  3. Ongoing and continuous services and linkages (including monitoring and evaluation)

Individualized treatment is utilized to facilitate community living and psychosocial rehabilitation. Mobile and accessible, ACT team members provide services in the client's living environment. Services are available to clients 24/7.
